Before the CARE adoption form is presented for completion, please read the following and:
  1) Click on the I Accept button at the end if you understand and accept the provisions.
  2) Click on the I Decline button at the end to terminate adoption form processing.
CARE Adoption Selection

  • For adoption requirements, fees, and breed selection guidance, be sure you read and understand the information at How To Adopt.
  • You must be 21 years of age or older to adopt an animal from CARE.
  • Note that submission of an adoption request does not guarantee that you will be selected as the new guardian for an animal. CARE fosters know their animals' behaviors and are trained to look for a good fit in placing animals. CARE wants to find the right home for each animal in their system. Many factors are considered, including: current pets, work schedules, children, yard/fence, etc. If you are not selected to adopt a desired animal, there may well be another animal better suited for your current situation.
  • CARE does not discriminate based on age, sex, gender, race, or any other such factor. CARE's only concern is finding the right home for an animal.
  • CARE is an all-volunteer organization. CARE foster homes make the best selections for new homes given the information and time available.
CARE Adoption Agreement

I understand that the following provisions and others will be a part of any adoption agreement I may enter into with CARE:
  1. If the pet is not already altered, I agree to have the pet altered according to the contract agreement as soon as it is medically possible.
  2. I agree to keep an identification tag attached to a properly fitted collar which will remain on the adopted animal at all times, whether inside or outside of the house, and to obtain all of the necessary city licenses required by local authorities.
  3. I agree to provide the adopted pet with necessary inoculations and heartworm preventative at the intervals advised by my veterinarian.
  4. I agree to have the adopted pet under my control when it is not on the confines of my property. A secured area will be provided for pets, including shelter from the elements. The adopted pet will not be tied or chained.
  5. If for any reason I cannot keep the adopted pet, I agree to notify the CARE of the availability of the pet and to return the adopted pet upon request.
  6. I agree not to mistreat the adopted pet and I authorize CARE, at their sole discretion, to determine whether or not the pet has been mistreated.
  7. I understand that any failure to perform the foregoing agreement would constitute a breach of contract. In the event of any such breach of contract, I authorize CARE to reclaim both possession and guardianship of the adopted pet.
  8. I understand that this pet is, as far as can be determined by CARE, in good health and that CARE is not responsible for any medical fees incurred after the adoption date. However, if a health problem develops during the first 10 days, I should notify CARE to discuss the matter.
  9. I agree to give CARE visitation rights to ensure that the terms of this adoption agreement are being observed.
