Before the Chatham County Animal Shelter adoption form is presented for completion, please read the following and:

- CARE accepts no responsibility for and has no knowledge of the behavior or
physical condition of any of the animals on the Chatham County Animal Shelter
section of our Web site.
- CARE makes no representations regarding the accuracy of the information
provided about the animals, has no information regarding the physical condition
of the animals or their behavior, and no one associated with CARE has otherwise
interacted with these animals in any official capacity on behalf of CARE.

I hereby agree to:
- Keep and maintain the animal in a humane manner and keep it in my home or on my property.
- Take the animal to a licensed veterinarian within ten (10) days from the adoption date for a medical examination.
- Have the animal spayed or neutered by a licensed veterinarian within ten (10) days from the date of this adoption
unless otherwise delayed by the veterinarian. In the event I do not have the animal spayed or neutered within the
specified time, the adoption fee will be forfeited and ownership of the animal shall revert back to CCAC.
- Provide any medical attention required or needed. This includes at least one annual medical check up, any
vaccinations indicated and preventative heartworm medication.
- Have the animal vaccinated for rabies as required by state law.
- Not give away or abandon the animal. If I find myself unable to keep it, I will return it to Chatham County Animal
Shelter.
- Hold Chatham County and CCAC harmless from any incurred fees or damages caused by the animal while it is in
my care.
- Give permission to the agents of CCAC to visit my premises at any reasonable time to insure compliance of this
contract.
Fees paid at the time of adoption include:
- First veterinarian's examination.
- An inoculation of dogs against distemper, hepatitis, leptospirosis, parainfluenza, coronavirus and parvovirus. An
initial vaccination for cats against rhinotracheitis, calicivirus and panleukopenia.
- The entire cost of spay or neuter.
Adoption Fee is refundable within the 10 days only if the animal requires major medical treatment or dies of natural causes. The refund does not apply if the animal is lost, stolen, injured or killed.
